ACTIVITIES Coordinator - Adult Day Care Services
Monday - Friday (No weekends or holidays!!)

Qualifications: Individual must have a high school diploma or GED with at least three (3) years of experience in activities coordination or therapeutic recreation for the aged, individuals with disabilities, or other special populations.
Good physical agility to be able to assist participants as needed and a sociable personality to conduct and engage participants in a variety of activities.
Reports to: Director of Adult Day Services
Responsibilities:
• Development and coordination of activities program designed to promote the Participant' active involvement with each other, their families and the community, and involve staff in implementing program goals.
• Demonstrate involvement with all Participant, staff, Participant' families, volunteers, and community resources.
• Maintain a written updated Participant' interest list and capabilities (Activity Assessment)
• Prepare a monthly calendar for planned activities in large print, and post in prominent location on first day of month and update as needed, before the activities take place.
• Include the following types of activities on the posted calendar:
• Social and Recreational Activities.
• Diversional and Intellectual Activities
• Work-Type and Volunteer Activities
• Trips and Outings in the Community
• Participate in evaluating the overall effectiveness of activities program at least once every six months.
• Documentation in MatrixCare AOD system of participant activity involvement every 90 days.
• Assist the Participant in transportation as related to the activities program when so indicated by the Director.
• Provide participants with any assistance needed in ADL's or supervise Program Assistants in the provision of such assistance.
• Take inventory and purchase activities supplies including recreational equipment, supplies for games, books, current magazines, and daily newspaper for living and recreational areas to operate within the Center's budget and according to administrative policy.
• Receipts for activity supplies must be signed and kept on file for Director.
• Help maintain the self-respect, personal dignity and physical safety of each resident.
• Report any accident/incident to Director or designee.
• Work cooperatively with all other staff and relate favorably to Participant and families.
• Be familiar with confidentiality of resident information and adhere to requirements.
• Be familiar with and able to apply all the center's accident, fire safety, OSHA, and emergency procedures.
• Be familiar with the Adult Day Care Center Participant' Bill of Rights and assist Participant with exercising these rights.
• Assure careful treatment of Adult Day Care Center equipment and utilization of supplies.
• Be responsible for reading and adhering to all Adult Day Care Center rules and regulations and employee policies.
• Participate in job-related continuing education as per Director's instructions.
• Perform other related duties as assigned by Director.
